                Duly sworn, I solemnly declare and affirm that at the time of issuing my birth registration my name was mistakenly written as Md. XXXXX Ali XXXXX, Father- M.A. XXXXX, Mother- Mst. XXXXX Nesa and dated of birth was inscribed as 28/07/1979 AD. Due to this I have been suffering a great difficulty and loss. Having taken you as a learned notary public, I being attended before you this day corrected my true name as Md. XXXXX Ali Chowdhury XXXXX, Father- Md. XXXXX XXXXX Chowdhury, Mother- Mst. XXXXX XXXXX and date of birth as 28/07/1978 by this affidavit. My name, father’s name, mother’s name and date of birth as corrected by this will be used and applicable everywhere in all cases and spheres from this day.
